Eldercare Concierge Services is designed to be a value-added resource for your employees or your customers and clients as they address the challenges of aging and eldercare. Offering this service helps your organization address eldercare needs with the goal of strengthening relationships and enhancing loyalty.

Eldercare Concierge Services are available at three levels. The services at each level can be tailored to best address the unique needs of your employees, client and customers.

Level 1:

  • Phone consultation with a professional Geriatric Care Manager
  • Information about and referral to community resources
  • Quarterly educational materials on eldercare topics

Level 2:

  • All level 1 services
  • Home safety assessment to determine potential risks and make recommendations for accommodations
  • Quarterly eldercare seminars that you can promote to your clients

Level 3:

  • All level 1 and level 2 services
  • Personalized assessment of an individual’s medical, cognitive and environmental needs
  • Detailed, written care plan that includes recommendations from the in-home assessment
  • Document and policy review
